Client Information

Residential care costs are funded by the Transport Accident Commission (TAC), WorkCover and Department of Human Services, through an Individual Support Package or other applicable funding bodies.

The residential care cost will vary depending on the level of support provided in the facility.

This funding is arranged and approved prior to admission.

In addition to this, client's daily living costs are charged which includes rent, utilities and food.

Some clients may be able to gain full or partial rent assistance through Centrelink to cover the client daily living cost.

Attendant care services are provided in the client’s home in Metropolitan Melbourne, regional Victoria, specifically Geelong, Bendigo and Shepparton, in Darwin in the Northern Territory and also in Albury, NSW.

Our post-hospital care program provides a range of services e.g. help with showering, shopping, meal preparation upon discharge from hospital to ensure the successful transition to home from hospital.

Healthscope Independence Services acknowledges the support of the Victorian Government.

An independence support plan is developed by our support workers for each individual client and outlines their needs, future goals and objectives.

Each individuals support plan is developed in collaboration with the client, their family and the client's therapy support team with a view to enhance the client's right to make their own decisions.

The independence support plan includes the client's short and long term goals relating to their personal care needs, rehabilitation and community access.

These goals also include an action plan to highlight how they will be achieved by the client.

The independence support plan is used as a daily working document for all of our carers and support workers.

The individuals plan is reviewed regularly to assess progress and review or reset the goals.

The recruitment of Disability Support Workers is directly linked to and determined by the requirements of individual clients.

We identify the skills required by the carers and acknowledge each client's specific requests, such as gender, age, interests and cultural background matching suitable carers to each client.

Our disability support workers are carefully selected and recruited based on the specific requirements of our individual clients. All staff are required to complete mandatory training annually as well as client specific training, as required.

Our support workers are trained and experienced in the areas of:

Personal Care

Attending to activities of daily living such as showering, dressing and grooming.

Community Access

Including recreational and leisure activities, accessing social networks, joining sporting clubs and pursuing hobbies in local communities.

Rehabilitation Support

Providing physical and psychological support to clients in collaboration with the client's therapy team which may include the fields of physiotherapy, occupational therapy and psychological therapy.

Goals are set within each Independence Support Plan to assist and enhance levels of independence.

Support workers provide therapy support under the guidance of their client's own therapist and receive client specific training from these therapists.

Acquired Brain Injury

General training is provided for support workers in addition to helping achieve individual client's Independent Support Plan (ISP) needs.
